These forms are used to initially enroll children and provide an annual check of data on file (required by Children & Family Services, others) for each child. For new enrollment a "print blank record" call is made in the print window. Annual records are printed as a batch with a "print current records" call to the print window.

The beauty of this form is that immediate data correction can be posted directly TO the database from the form as returned by the parents. Allows tracking of initial registration fee payments (if applicable). Plus, parents must sign or decline permissions on a current basis. All important data like phone numbers, names of alternates, etc. is tracked.
