This is the "Parent/Guardian Information" section, the second section to be entered to complete a child's record entry process. Credit and collections problems are smartly handled by parent full name, social security, dirvers license, and work data. The rule is basic - failure to supply this data will abort the parent effort to enroll the child - it is also suggested you make a copy of mom & dad's drivers licenses to: 1) identify new parents when picking up new enrollees, and, 2) as a powerful tool to assist bad debt collection efforts with the local district attorney. Many states can prohibit license and auto registration renewals with this information. And besides, you would normally write drivers license information on personal checks anyway.
